Family Support Program, a family-centered approach in the home
Service Description 

Case managers work with families to identify needs and goals to help them to succeed. Personalized visits take place in homes, schools, and office locations. The program assists with the referral and application process to community resources to address needs, including food, shelter, child care, and medical care.
